When you receive the reactivation agreement contract from us please complete the information requested. Return the original to CCR in the postage paid envelope provided.

Upon receipt of your signed contract we will process the case immediately and notify you on the status of you claim. In some cases, more information will be required to confirm Rightful Ownership. If this should occur we will contact you with instructions.
